Interleukin-4Rα (IL-4Rα), der Hauptrezeptor von IL-4, hat sich als "inspirierendes" Ziel für innovative Therapien zur Behandlung verschiedener entzündlicher und onkologischer Erkrankungen erwiesen. IL-4Rα ist ein Typ-I-Transmembranprotein, das eine Schlüsselrolle in jenem molekularen Weg spielt, der ein spezifisches Immunmuster, die sogenannten Typ-II-Entzündungen, antreibt.
Dieser Signalweg wird von zwei Zytokinen gesteuert: Interleukin-4 (IL-4) und Interleukin-13 (IL-13). Beide Zytokine haben vielfältige biologische und immunologische Auswirkungen auf die Anpassungsfunktion von Lymphozyten, dendritischen Zellen und Fibroblasten. Insbesondere binden diese Zytokine an IL-4Rα, um den Entzündungsweg des Typs II in Gang zu setzen, der die Differenzierung von Th2-Zellen, die Entzündung der Atemwege und die Schleimproduktion umfasst.
Klinische Anwendung
IL-4R und die Zytokine IL-4 und IL-13 sind wichtige Regulatoren der humoralen und adaptiven Immunität. Dies bedeutet, dass ein Ungleichgewicht entweder in der Rezeptorexpression oder in den zirkulierenden Signalzytokinen klinisch beobachtet werden kann. Die daraus resultierende Überreaktion des Immunsystems wird durch ein anfängliches Ungleichgewicht der TH1- und TH2-Differenzierung ausgelöst, das eine abnorme Sekretion von Zytokinen bewirkt. Aktivierte Th2-Zellen setzen Zytokine wie IL-4, 13 und 31 frei, die nachgeschaltete B-Zellen zur Umwandlung und Produktion von Immunglobulin E (IgE)-Antikörpern aktivieren. Im Gegenzug werden Mastzellen und Basophile rekrutiert, die degranulieren und Entzündungsfaktoren produzieren. Gleichzeitig binden die sezernierten IL-4 und 13 weiterhin an ihre jeweiligen Rezeptoren (z. B. IL-4R), was wiederholt die TH2-Differenzierung und die anschließende Entzündung fördert. Diese Wirkung wird bei verschiedenen Autoimmunkrankheiten wie Asthma, Ekzemen und Heuschnupfen beobachtet.
Asthma
Asthma ist eine komplexe, anhaltende und lebenslange Entzündungskrankheit. Dupilumab ist ein humaner monoklonaler Antikörper, der auf IL-4Rα abzielt. Sein Wirkmechanismus besteht in der spezifischen Blockierung des Typ-II-IL-4R/IL-13R-Signalwegs bei gleichzeitiger Hemmung der intrazellulären Aktivität von IL-4 und IL-13 mit STAT6. Durch die Unterbindung der Zytokinsignalisierung an der Quelle wird die Th2-Entzündungsreaktion minimiert.
Ekzem
Ekzem, auch bekannt als atopische Dermatitis, ist eine Krankheit, die durch Hautentzündungen, trockene Haut, Ausschläge und Blasen gekennzeichnet ist. Neuere Studien weisen darauf hin, dass Ekzeme möglicherweise auf eine Überexpression der IL-13/IL-4-Signalübertragung bei bestimmten Personen zurückzuführen sind. Daher ist die Entwicklung von Therapien, die auf den IL-13/IL-4/IL-4R und seinen intrazellulären Signalweg abzielen, eine vielversprechende innovative Strategie.
Heuschnupfen
Heuschnupfen, auch bekannt als allergische Rhinitis, ist eine häufige chronische Erkrankung, die durch eine IgE-vermittelte Entzündungsreaktion in der Nasenschleimhaut ausgelöst wird und eine Vielzahl von Komplikationen verursacht. In einer kürzlich durchgeführten Studie konnte gezeigt werden, dass die Hemmung des IL-4/STAT6/GATA3-Signalwegs die IL-4-Sekretion, das Serum-IgE und die Schleimproduktion in den Atemwegen verringern kann, wodurch sich die Heuschnupfensymptome wirksam verbessern.
Metastasierende Tumore
Neben Autoimmunkrankheiten ist IL-4R auch bei vielen Epithelkarzinomen überexprimiert. Die intrazelluläre Signalübertragung von IL-4/ IL-4R in Lymphozyten steuert hingegen den Entzündungsweg vom Typ II. Die Überexpression von IL-4R treibt also die Metastasierung und das Tumorwachstum voran, was IL-4R hemmende Medikamente als potenzielles therapeutisches Mittel für metastasierende Tumoren vermuten lässt.
